How to Register

PhilHealth YAKAP Yaman ng Kalusugan Program

Requirements

For Registered and Non registered YAKAP Clients: Livebirth (for Non registered dependents only)
Valid PhilHealth Identification Number (PIN) or PhilHealth ID
Proof of eligibility (active PhilHealth membership)
Valid government issued ID of member or dependent
Medical prescription or diagnostic request (if applicable)
Smartphone with eGov PH (for digital registration) and active Email Address

Get a queue number at the front table
1. Secure a PhilHealth Identification Number (PIN) (if not yet a member).
2. Register with the PhilHealth YAKAP Program
3. Visit the Primary Care Facility for the first consultation.
4. Undergo laboratory tests or diagnostic procedures as recommended by the YAKAP Accredited Physician.
5. Avail of prescribed YAKAP medicines.
6. Return to the YAKAP clinic for scheduled follow up consultations.
